The Trinity Center at Hardeman County Memorial Hospital is an intensive outpatient mental health program designed for senior adults. Our program gives older adults the opportunity to explore, address, and find healthy alternatives for coping with the stressors that life can bring — grief, loss, depression, anxiety, and the changes that come with aging.
Our philosophy is simple: we help individuals identify the emotional barriers standing between them and a healthier, more positive life — and we walk alongside them as they make those changes. The journey to emotional well-being does not need to be made alone. The Trinity Center provides the safe, warm, compassionate, and therapeutic environment needed for real change to take root.

How to Make a Referral
Anyone can make a referral to the Trinity Center — a family member, a caregiver, a friend, a primary care provider, or the person themselves. Only a physician, however, can determine whether services are clinically appropriate, so part of our intake process includes coordinating with your doctor.
